DATA SUBJECT'S EXPLICIT CONSENT DECLARATION FORM

Pursuant to Law No. 6698 on the 'Protection of Personal Data,' I hereby accept, declare, and undertake that I consent to the processing of my personal data, sensitive personal data, and contact information by Albatross Marine Services; including all kinds of personal data, whether provided verbally/in writing and/or electronically, identifying or enabling the identification of my identity and contact details, to be processed, and to be shared for commercial purposes with sponsor companies that require sharing within the scope of the relevant legislation. I consent to the processing of my personal and contact data as defined in the Law No. 6698 on the Protection of Personal Data, as detailed below.

- Data that clearly belongs to an identified or identifiable natural person, processed wholly or partially by automatic means or otherwise as part of a data recording system; data containing personal identity information such as name and surname.

- Data that clearly belongs to an identified or identifiable natural person, processed wholly or partially by automatic means or otherwise as part of a data recording system; information such as telephone number, address, e-mail address, fax number, IP address.

- Data that clearly belongs to an identified or identifiable natural person, processed wholly or partially by automatic means or otherwise as part of a data recording system; records and documents obtained during entry to physical premises or while remaining inside physical premises; camera recordings, fingerprint records, and records taken at security points.

- Data that clearly belongs to an identified or identifiable natural person; photographs and camera recordings (excluding those falling under Physical Space Security Information), as well as data contained in copies of documents bearing personal information.

- Data that clearly belongs to an identified or identifiable natural person, processed wholly or partially by automatic means or otherwise as part of a data recording system; any requests or complaints submitted to Albatross Marine Services.

Within the scope of the Law No. 6698 on the Protection of Personal Data, I hereby accept, declare, and undertake in advance that I consent to the collection, recording, processing, storage, and commercial sharing of my personal data and contact information by Albatross Marine Services, in accordance with the principles set forth by law, with sponsor companies and stakeholders determined by the Board of Directors.
